clarification please

just trying to wrap my head around some of the stuff in this codex.. and I'm finding some of it isn't really 100% clear

for example with the devastator squad it says

Up to four Space Marines may replace their boltguns with one of the following:
- a heavy bolter, multi-melta or missile launcher +15 points
- a plasma cannon +25 points
- a lascannon +35 points
---

what I'm not finding very clear there is if it's 15 points per model or just 15 points.. I would think it would be per model.. that would make sense to me.. but I've seen other things actually mention some many points per model specifically.. so a little clarification would be most helpful for me.

That would indeed be per model.

So if you were to take 2 Heavy Bolters. a Plasma Canon and a Lascanon it would be an extra 90pts.
